Does it seem like many things that you learned about documentation, treatment planning, and goal setting are distant memories? Maybe you need a refresher on these important, yet sometimes overlooked, areas of clinical documentation. This workshop will prepare learners to develop methods of structured and effective documentation using acronyms such as SOAP, BIRP, and DAP. The fundamentals of treatment planning and goal setting using the SMART model will be discussed, and learners will be challenged to develop treatment goals as practice in the training.

• Develop methods of structured and effective clinical documentation that adhere to appropriate
standards.
• Identify three creative methods for helping clients develop goals and formulate SMART treatment
plans.
• Discover ways to engage in ethical practices to produce quality documentation and effective treatment planning with clients.
